Return Merchandise Authorization Form (RMA Request)

Before returning any item to Andrea Systems, LLC for repair evaluation, please obtain an RMA number by completing the form below. Please include the part number and serial number of the item. Once you are assigned an RMA number, please be sure to write this number on the outside of the carton so that we can ensure it is properly processed once it arrives at our facility. The RMA number must also appear on any paperwork that you enclose with the unit. Thank you.
